PSK (polysaccharide-K, sold in Japan under the brand name Krestin) and PSP (polysaccharopeptide) are two purified compounds extracted from the Turkey Tail mushroom (Trametes versicolor, also called Coriolus versicolor). Japanese researchers developed PSK in the 1970s, and Chinese researchers developed PSP in the 1980s. Both have been studied for decades as an add-on to standard cancer treatment, mainly chemotherapy, not as a replacement for it.

- PSK (Krestin): Developed in Japan in the 1970s. Manufactured as a standardized pharmaceutical-grade extract. Approved by Japan’s health ministry as an adjunct cancer drug.
- PSP: Developed in China in the 1980s from a different Turkey Tail strain (COV-1). Studied mainly in Chinese clinical research and used there in some hospital settings, but does not carry the same drug approval history as PSK.
Both are polysaccharide-peptide complexes, meaning long sugar chains bound to protein. Researchers believe they work by stimulating parts of the immune system, including natural killer cells and T-cells, though the exact mechanism in humans is still being studied. The Japanese Clinical Trial Evidence for PSK
According to the National Cancer Institute, medicinal mushroom extracts including PSK have been used as an addition to standard cancer treatment in Japan for more than 30 years. The evidence base is larger and older than most people expect.
- A randomized trial of 751 patients with gastric (stomach) cancer, conducted from 1978 to 1981, found that patients who received chemotherapy plus PSK after surgery lived longer than patients who received chemotherapy alone.
- A 1994 Japanese study of 262 gastric cancer patients found reduced cancer recurrence and improved survival in the group that received PSK alongside chemotherapy.
- A review of randomized controlled trials in breast cancer, covering more than 2,200 patients across Japan and Korea, found that PSK added to standard treatment significantly extended survival in one specific subgroup: patients with ER-negative, stage IIA breast cancer without lymph node involvement.
- Additional Japanese trials have tested PSK as an adjunct in lung and colorectal cancer, generally comparing chemotherapy alone against chemotherapy plus PSK.
The National Cancer Institute also notes that few side effects have been reported in PSK studies conducted in Japan, where it has a long history of use as a prescribed adjunct therapy.
The Chinese Research on PSP
PSP has a smaller but overlapping evidence base, developed largely through Chinese academic and hospital research starting in the 1980s. Chinese researchers have studied PSP as a supportive therapy alongside chemotherapy and radiation in several cancer types, with a research focus on quality of life, immune cell counts, and symptom burden during treatment, in addition to survival outcomes. This body of work is smaller in scale than the Japanese PSK trials and has not led to a formal drug approval outside China.

The United States Food and Drug Administration has not approved Turkey Tail, PSK, or PSP as a treatment for cancer or any other medical condition. This is not a case of the evidence being ignored. It reflects different regulatory systems and different evidentiary standards for what counts as an approved drug.
A key practical point: the FDA does not regularly review how dietary supplements are manufactured for potency or consistency. That means a Turkey Tail supplement bought online or in a store has not gone through the same testing, purification, and quality control as the pharmaceutical-grade PSK used in Japanese trials.
Human Clinical Evidence vs. Preclinical and Mechanistic Findings
