Mushrooms and Immune Function: Cross-Species Research Summary

Turkey tail is the mushroom with the most human clinical trial evidence tied to immune-related outcomes, mainly in cancer patients taking it alongside chemotherapy. Reishi has the most laboratory and animal evidence, but human studies are smaller and more mixed. Shiitake and maitake sit further behind, with mostly preclinical and mechanistic research. No mushroom species has been proven to prevent illness or replace medical treatment in a healthy person.

Eating raw or undercooked shiitake mushrooms can trigger a striking, itchy, whip-mark-shaped rash called shiitake flagellate dermatitis. It usually appears 24 to 48 hours after eating the mushroom, is not an emergency, and typically clears on its own, but it can look alarming and is worth knowing about before you try shiitake.

Reishi may lower blood pressure, lower blood sugar, and slow blood clotting. Combining it with medicines or supplements that do the same things may increase the risk of low blood pressure, low blood sugar, or bleeding. People should stop high-dose reishi at least two weeks before any scheduled surgery and talk with a clinician first if they take blood pressure medicine, diabetes medicine, or blood thinners.

How Scientists Study Mushrooms and the Immune System

Researchers mostly focus on a group of compounds called beta-glucans, which are large polysaccharides found in mushroom cell walls. In lab and animal studies, beta-glucans interact with immune cells such as monocytes, natural killer (NK) cells, and dendritic cells. That is a mechanism, not proof that eating a mushroom or taking a supplement changes a person’s immune response in a meaningful way. Only human clinical trials can show that.

Species-by-Species: Human Data vs. Preclinical Data

Turkey Tail (Trametes versicolor), and Its Extract PSK

  • Human evidence level: the strongest of the mushrooms covered here, but almost entirely in cancer patients, not the general public.
  • PSK (polysaccharide-K), an extract from turkey tail, is an approved adjunct cancer therapy in Japan and has been used alongside chemotherapy in thousands of patients since the 1970s.
  • A meta-analysis of randomized trials in colorectal cancer patients linked PSK plus chemotherapy to better 5-year disease-free and overall survival than chemotherapy alone.
  • A meta-analysis covering more than 8,000 gastric cancer patients found a modest survival benefit when PSK was added to chemotherapy after surgery.
  • Trials in lung cancer patients reported benefits in at least one measured outcome — such as immune function, body weight, or survival — when PSK was combined with standard chemotherapy or radiation.
  • Important limit: this research was done in people already being treated for cancer, on top of chemotherapy or radiation. It does not show that turkey tail prevents illness or “boosts” immunity in a healthy person.

Reishi (Ganoderma lucidum)

  • Human evidence level: limited and mixed. Most of the strong evidence for reishi comes from cell and animal studies, not human trials.
  • In one clinical study, 30 people with advanced lung cancer took a reishi polysaccharide extract for 12 weeks. Their immune cell counts and immune signaling markers did not change in a statistically meaningful way compared with before treatment.
  • A Cochrane systematic review of reishi for cancer treatment concluded the evidence was not strong enough to recommend reishi as a treatment on its own.
  • A 2024 survey of more than 1,300 cancer patients using reishi found many reported feeling less fatigue, nausea, or poor appetite, but these were patient-reported feelings, not measured immune changes, and about 9% of users also reported side effects like dry mouth, constipation, or insomnia.
  • According to a U.S. National Library of Medicine review of the evidence, there isn’t enough reliable research to say reishi is effective for most of the purposes people use it for, including general immune support.

Shiitake (Lentinula edodes)

  • Human evidence level: limited for immune claims specifically. Most detailed clinical use has focused on lentinan, a compound extracted from shiitake.
  • Lentinan has been studied in Japan alongside standard treatment for stomach and colorectal cancer, according to the National Cancer Institute’s cancer terms reference.
  • Whole-food shiitake research has focused more on cholesterol and general nutrition than on measured immune outcomes in humans.
  • The clearest, best-documented human effect of shiitake is not an immune benefit at all — it’s the flagellate skin reaction described above, which is a known reaction to raw or undercooked shiitake, not to the immune system.

Maitake (Grifola frondosa) and Other Species

  • Human evidence level: preliminary. Maitake is grouped with reishi, turkey tail, and shiitake as one of the most-studied medicinal mushrooms, and it shares the same general beta-glucan mechanism researchers study in the others.
  • Compared with turkey tail and reishi, published human clinical trial data on maitake and immune outcomes specifically is thinner. Most of what’s available focuses on laboratory and animal research.
  • Other species sometimes marketed for immunity, such as chaga and cordyceps, are not covered by the sources used for this article and are not addressed here.

  2. Look for the word “human.” A study description that only mentions cells, mice, or rats has not been shown to apply to people yet.
  3. Check who the study participants were. Evidence from cancer patients on chemotherapy does not automatically apply to a healthy adult trying to avoid a cold.
  4. Look for the dose and how long people took it. Marketing claims rarely match the amount or duration used in the actual study.
  5. Ask a pharmacist or clinician about interactions if you take any prescription medicine, especially for blood pressure, blood sugar, or blood clotting.

Evidence Limits You Should Know

Most of the strongest human trials described here were conducted in Japan or China, often in people already diagnosed with cancer, and often studying a specific processed extract rather than a raw mushroom or a typical over-the-counter capsule. Results in that setting do not automatically transfer to a general immune-support supplement bought online. In the United States, the FDA does not evaluate dietary supplements for effectiveness before they’re sold, and product quality and ingredient amounts can vary between brands and batches.

Who Should Use Extra Caution

  • People who are pregnant or breastfeeding, since there isn’t enough reliable safety information for reishi in this group.
  • People with bleeding disorders, or anyone with surgery scheduled in the next two weeks, because reishi may slow blood clotting.
  • People taking blood pressure medicine, diabetes medicine, or blood-thinning medicine, due to possible added effects with reishi.
  • Anyone with a known mushroom or fungal allergy.
  • Anyone currently in cancer treatment, who should talk with their oncology team before adding any mushroom product, since some of the human research involves specific extracts used under medical supervision.
